Tirzepatide activates GIP and GLP-1 receptors. That makes it a different medicine from semaglutide, but the practical comparison requires more than the mechanism. Its use should be considered through the specific product label, relevant trials, and the person’s health circumstances.
Brand and jurisdiction change the approved use
In the US, Zepbound is approved for adult weight management and for moderate-to-severe obstructive sleep apnea in adults with obesity, alongside diet and physical activity. The current US label describes weekly subcutaneous administration. In the EU, the Mounjaro record includes diabetes and adult weight-management uses. These are independent jurisdiction-specific records. US Zepbound label and EMA Mounjaro record.
What obesity trials add
SURMOUNT-1 established substantial average weight reduction with once-weekly tirzepatide in adults with obesity or overweight without diabetes over 72 weeks. Its population and treatment schedule belong beside any percentage quoted from the trial. It does not establish that the largest trial average is the appropriate expected result for every user. SURMOUNT-1 trial.
SURMOUNT-5 provided a direct randomized comparison with semaglutide in adults with obesity without diabetes. Tirzepatide produced greater average weight reduction at week 72 in that study. It was open-label and compared specified maximum-tolerated dose ranges. It did not compare tirzepatide with every semaglutide formulation or dose later approved. SURMOUNT-5 paper.
An indication for sleep apnea has a defined scope
FDA’s sleep-apnea decision applies to adults with obesity and moderate-to-severe obstructive sleep apnea. It was based on controlled studies, including participants using positive airway pressure and those unable or unwilling to use it. An approval is not an instruction to stop prescribed airway treatment on your own. Discuss changes with the team managing the condition. FDA sleep-apnea announcement.
Consider safety and the plan for follow-up
The US product label contains a boxed warning about thyroid C-cell tumors and other important restrictions. A treatment discussion should also cover gastrointestinal symptoms, other medicines, pregnancy considerations, and planned procedures. Those issues cannot be resolved by selecting whichever study had the larger average weight change.
For an appointment, write down what you want treatment to improve and what might make continuing difficult. A useful comparison considers tolerability, access, and follow-up alongside efficacy. Ask how the care team will decide whether the benefit remains worthwhile, and how they will handle any treatment interruption.
The evidence supports defined uses in defined populations. It does not establish that everyone needs a higher dose, that two medicines should be combined, or that a brand used for one indication can be substituted without a prescribing decision.
